What is a Change of Circumstances?


If your situation changes including your health, living arrangements or support requirements – you can notify the NDIS by submitting a Change of Circumstances.

This will allow you to utilise a NDIS Plan that is current with your goals and ensuring you are getting the right level of support.

REQUIREMENTS FOR A CHANGE OF CIRCUMSTANCE:


Complete the NDIS Form


Head onto the NDIS Website and Complete the ‘Change of details or Change of Situation Form’.

The form outlines the Participants, Plan Nominee and Support Coordinator’s Contact Details and will ask further detailed questions regarding the Change of Situation.

Provide Evidence/Supporting Documentation

It is beneficial to obtain as much Supporting Documentation as you can!

The Supporting Documentation will give the NDIA a further insight around your needs and gain a better understanding of what exactly has changed and why extra supports are required.

Supporting Documentation can be provided by:

-          Support Workers

-          Allied Health Professionals

-          GP/Specialists

-          Plan Nominee


Functional Capacity Assessment (FCA)


A Functional Capacity Assessment or FCA is a detailed report completed by an Occupational Therapist that will assess the Participant’s needs and complexity of their Disability.

 

A Functional Capacity Assessment will assess all areas of a Participant’s Life and will provide a Professional Opinion to what has changed and what further supports are reasonable and necessary.

 

A Functional Capacity Assessment can typically take between 6 to 12 hours, depending on the Complexity of the Participant’s needs. This report can typically be funded from the Participant’s Improved Daily Living Budget.


Helpful Tips:


If there has been a change in your circumstances relating to Guardianship Orders, Power of Attorney, Child Protection or Court Orders – be sure to include this documentation to your Change of Circumstances Submission.

Your Support Coordinator or Local Area Coordinator (LAC) can assist you with your Change of Circumstance Submission.
